Durham County Council is seeking a supplier for a Housing, Health & Wellbeing Hospital Discharge Support Service to prevent individuals aged 18 and over being discharged from hospital into street homelessness and to support safe, timely and sustainable discharges through coordinated in-hospital intervention and time-limited post-discharge support. The service must deliver early identification of people who are homeless or at risk, in-hospital assessment, advocacy and discharge coordination, and time-limited post-discharge assistance to sustain accommodation and reduce the risk of readmission, specifically supporting people with drug and alcohol support needs and those known to Adult Social Care. Providers must operate within the Care Act 2014 framework, support duties under the Homelessness Reduction Act 2017, work as part of an integrated model with Adult Social Care, housing and health partners, accept referrals from Transfer of Care Hubs, hospital wards, Emergency Departments and clinical teams, and be prepared to work flexibly and co-locate with other commissioned services where required; further requirements are set out in the ITB documents available at www.open-uk.org.
