Stoke-on-Trent City Council and Staffordshire and Stoke-on-Trent Integrated Care Board are seeking providers within the local Voluntary, Community, Faith and Social Enterprise (VCFSE) sector to deliver the Better Care Fund-funded Integrated Wellbeing Pathways service for adults aged 18+. The contract is proposed for 3 years with the option to extend for two further 1-year periods (3+1+1), with an estimated annual contract value up to £557,282.00 and a maximum potential value of £2,786,410.00 over the full term. The service is split into two lots: Lot 1 Community Support and Development (approx. £285,282 per year, with a minimum of £30,000 ring-fenced) delivering predominantly home-visit, responsive short-term wellbeing support, community development and seven-day services; and Lot 2 Integrated Hospital Discharge and Neighbourhood Support Pathway (approx. £272,000 per year, with an agreed nominal £105,000 ring-fenced) focused on safe, timely hospital discharge, rapid access to community/VCFSE/Personal Health Budget support, post-discharge follow-up and seven-day neighbourhood coordination. Across both lots providers will help people live independently, improve health and wellbeing, avoid unnecessary admissions, build connections and strengthen local community capacity. Providers must have strong links with local communities and referral partners, experience of collaborative working and the ability to share outcomes and performance; submissions from individual organisations, partnerships, consortia or lead provider/subcontractor arrangements are welcome.
