The service aims to improve quality of life, promote independence and wellbeing, reduce health inequalities, enhance care coordination across health, social care and voluntary sectors, prevent avoidable crises, hospital admissions and delayed discharges, and embed lived experience into service design, delivery and evaluation while maintaining person-centred practice and clear referral pathways that consider geographical spread, demographic needs and dementia prevalence across Birmingham and Solihull.
