NHS South West London Integrated Care Board (ICB) (Croydon Place) is seeking a supplier for Long-Term Conditions (LTC) community outreach, the Expert Patient Programme (EPP) LTC self-management courses and Pentathlon healthy lifestyle programme services to support prevention, early identification and improved management of LTCs in Croydon, targeting people who do not regularly access GPs or local NHS services. Since 2020 these programmes have aimed to raise awareness of LTC risk and support healthier lifestyles, early diagnosis and self-management, with a focus on cost-effective, culturally competent interventions that address health inequalities in Black, Asian and Minority Ethnic communities (which make up around 50% of SWL ICB's core20 communities) and in deprived wards such as Thornton Heath and New Addington. The contract is expected to start in April 2026 for an initial two-year period, with a possible one-year extension at the authority’s discretion, and a total value of £297,000 inclusive of VAT (£99,000 per annum inclusive of VAT). This is a regulated below-threshold, light-touch procurement governed only by the applicable provisions of the Procurement Act 2023.
